Choose the form based on the age of the person changing their name.
If the person is 16 or over
Complete and return the over 16 application form.
People over 16 can only have:
- one forename change
- up to 3 surname changes, with a gap of at least 5 years between each
If the person is under 12 months old
Complete and return a form for either:
People under 16 can only have:
- one forename change
- one surname change
If the person is over 12 months and under 16 years old
Complete and return the under 16 application form.
People under 16 can only have:
- one forename change
- one surname change

Processing times
We’ll aim to email you within 20 days of receiving your application to:
- confirm the details of the change
- request payment
After you pay, we’ll email you within 15 working days to:
- confirm when the change is complete
- explain how to order a replacement birth certificate
