If you apply for more than one family member at the same time, it costs an extra £10 for each additional person.
After we receive your application, we’ll email you with instructions on how to pay.
You must include an email address with your application. We use email for all future contact.
We do not automatically provide a replacement birth certificate. We’ll explain how to order one after the change is complete.
